Job Description

Summary

The Hospice Regional QAPI Coordinator is accountable for management and coordination of an integrated Quality Assessment and Performance Improvement program in the assigned region. This position is responsible for the on-going preparation for certification and licensure surveys for LHC Group Hospice locations within the region. Worked hours may vary with after work hours and travel. Must be willing to travel, on a scheduled and emergency basis as needed, to different Hospice locations. This
person must be able to anticipate needs of the region and take appropriate action.

Remote position with approx. 50% travel.

Territory to be assigned based upon where candidate resides.



Responsibilities
  • Oversees Quality Assessment and Performance Improvement projects identified through the on-going Hospice data driven quality assessments and performance improvement programs for assigned Hospice Agencies. These programs include but are not limited to clinical outcomes, internal process measures and family satisfaction.
  • Keeps abreast of state and federal regulatory requirements and coordinates education for QAPI principles and regulatory standards for hospice staff.
  • Serves as a resource to medical staff, administrators and other staff of assigned Hospice Agencies. Keeps abreast of issues within each agency, providing support to administrators and staff as needed to facilitate efficient operation and quality patient outcomes.
  • Responsible for Regulatory Survey Preparedness at assigned Hospice Agencies. Ensures that assigned Hospices are operating within individual state and federal guidelines appropriate to the agency.
  • Identifies needs and concerns through review of Hospice CAHPS and Hospice Item Set data and helps agencies to implement corrective action plans through the QAPI process. Monitors sustainability of improvements through the agency data collection processes.
  • Plans and implements in-service and continuing education based on QAPI findings to meet the training and educational needs of the hospice staff.
  • Conducts on-site surveys of branches to assess compliance with meeting regulatory, accreditation, and payer source guidelines.
  • Assists the Hospice QAPI manager with and specific QAPI related task such as research and development of educational material for identified in-service, etc.
  • Assists the agency with completion of various required reports such as the agency action plan, Quarterly Executive Summary, State Survey Plan of correction, etc.
  • Other duties as assigned by the Hospice Quality Manager.
  • Assist agencies in Joint Commission activities including survey preparation, survey response, and Focused Standards Assessment activities. Serves as a resource for guidance and questions.


Education and Experience
License, Registration, and / or Certification Requirement:
Education Requirements
  • A minimum of 5 years of health care experience. Hospice experience preferred.
  • Bachelor's Degree in a health care related field, Master's preferred.
  • Associate's Degree acceptable for RN candidates only.
License Desired
  • Certification in HealthCare Quality is preferred, after first year of service.
Skill Requirements
  • Understanding of CMS Conditions of Participation, Joint Commission Standards and state licensing standards.
  • Computer literacy, especially in Hospice Patient Management Systems, Excel, Word, and PowerPoint.
  • Effective presentation and communication skills; ability to present information in a clear and concise manner to large groups of people.
